Dear Tenant,

Please ensure that you read and clearly understand the terms and conditions contained herein:

1. Fee: $25.00 plus HST (Non-refundable deposit for the use of access card)
2. Any lost, stolen, or damaged access cards will be the tenant's responsibility to replace using this online form. Properly Management will receive and process with the same fee associated.
3. The holder of this access card cannot assign or transfer without the prior written consent of property management.
